Overview
You can create a Triggered Email that is sent when a customer gives a particular Answer to a Form Question that is presented during the Checkout process. For example, if the Event is one in which food is being served, the Form question may ask the customer which food option they prefer. The answers would then generate an email notification to an Administrator, or Coordinator letting them know how much of each food option to prepare.
This type of Triggered Email is system wide -- it does not need to be assigned to anything. You can have multiple of this type of Triggered Email in your database.
Creating the List of Answer Choices
Login as an Administrator on the database, navigate to Settings > Assignments > Lists and click the Add New List button.
Input a List Name and optionally, a Label, then click the Save button.
You may either click the Add New Item button, or Upload Items button. In this example, we are going to add new items.
Input a Title and Value, then click the Save button.
Repeat this process for each item, adding a New List Item for all of the Answer choices.

Creating the Form
Navigate to Settings > Assignments > Forms and click the Add New Form button.
Input a Title for your Form and choose the Form Type. For our example below, we will be using the Product Participant Form. This type of Form is displayed during Checkout, is connected to a Product and is about the Participant. Since the Account holder will complete the Form, we set the Visibility field to "Updatable by Account" and leave Pre-populate Answers for Faster Checkout as the default "Pre-populate answers". In case a similar Product in the future uses the same Form, the answer will be pre-populated. Click the Save button when ready.
You will then be able to compose your Form Question. Here, we are going to create a Radio Button List Form Question.
On the Edit Question screen, input a Title and compose the Question. In the example below, the question is Required. This means the Account holder must submit an Answer. An Administrator, when Impersonating, will not have to answer the question because Impersonator Skip has been set to Yes. If an Administrator is placing an Order on behalf of a customer, they do not need to answer. This way, they may still complete the Order. Also, the "Dinner Options" List of Answers that were created earlier is being used. When ready, click the Save button.

Creating the Email Message
Login to the Administrative side of your database, and navigate to Settings --> Assignments --> Email Messages. Click to Add New Email. Input a Title, choose the type Triggered Product Participant Purchased, and click the "Save" button.
You can now begin to compose your Email Message. Input a Subject, and add text to the Body. You can use plain text, and you can use Operators. Operators read data from the system that is relevant to the Order, Account, Form, or Product, and input that data into the email. For example if you are creating a message about that an Account placed an Order with a Form and gave a Form Answer to a Form Question, you would choose and organize Operators as in the example below. After you are finished composing your email message, click the "Save" button.
Creating the Triggered Email
Navigate to Settings --> Assignments --> Triggered Emails. Click the "Add New Triggered Email" button.
Main Fields
Input a Title, choose the Type, select your Message, choose the date range that the Triggered Email will be used, and choose what will trigger the sending of the Email Message. If you want the Triggered Email to not expire, you can select No End Date.
Send Fields
The trigger is fired off of an Answer to a Question on a Form. So, in our example, we will have to create a Triggered Email for each Answer choice.
For example, you can send an email to Assigned Coordinator when the customer choose Steak.
Additional Triggered Emails will have to be created for the Fish and Vegetarian options.
